HOW ENQUIRIES ARE HANDLED

Privacy notice.

Pre-launch notice: Practice Notes is a working service concept. Before publishing on a live domain, the operator must confirm its legal identity, business contact and operating privacy practices. The current pilot enquiry operator is proposed to be Maximalist Agency, contact rishabh@maximalist.agency.

Information collected

When you submit the enquiry form, it asks for your name, work email, clinic name, city, professional website or public profile, recording preference, interested service and an optional description of your content needs. Campaign parameters in the URL may be stored for attribution.

Purpose

This information is used to review fit, respond to your enquiry, discuss a proposed content pilot and maintain a basic record of enquiries. Submitting the form does not subscribe you to a newsletter or authorize unrelated promotional messages.

No patient information

Please do not submit patient names, medical histories, contact details, photographs, identifiable testimonials or other health information. The form is designed for business contact details only.

Storage and transfers

Enquiries are stored in a private server-side file. If the website owner configures an external customer relationship tool or automation webhook, the same business enquiry data may also be sent to that service. The website does not use analytics or advertising cookies by default. Actual hosting location and processor details must be disclosed before public launch.

Retention and requests

Enquiries should be reviewed and deleted when they are no longer required for their purpose, subject to applicable record-keeping duties. Requests to access or delete enquiry information can be directed to the operator's confirmed contact address.

Safety and accuracy

No online system can guarantee absolute security. Access to stored enquiry data must be limited, backups protected and software updated. This notice should be reviewed for applicable Indian privacy requirements before launch.
